HOME MEDICAL ASSESSMENTS

Geriatric assessment is designed to evaluate an older person's functional abilities, general physical health, cognition, and mental health as well as social circumstances for evaluating daily activities like hearing, balance, cognition, among other important geriatric assessment.

A complete assessment is initiated most likely when an elderly has a potential problem such as confusion, falls, incontinence, or mobility issues. A geriatric assessment is different from a standard medical evaluation because it emphasizes functional capacities and some potential problems are often related to different diagnoses. For example, a patient presenting confusion might not have a neurological problem but an infection.   

The purpose of a geriatric assessment is to identify any risk factors that an elderly patient might have that require further health management, in addition to elderly general health status, we identify a broad range of factors that can influence a senior´s physical, psychological, and social functioning.  

In addition to offering telehealth medical visits for minor or common illnesses, CCTC offers concierge in-home medical assessments for those encounters that may require direct observation or for more severe concerns.
